Enrollment
If you are currently enrolled in SelfDesign Global or in any SelfDesign-related program, then you only need to submit the course registration form, below. If you are new to SelfDesign, however, then please also submit the following school enrollment form:

All learners may register for courses by submitting the course registration form:

How to Submit
To work with these forms:
- Save file to your computer
- Open with Adobe Reader (version 7 or later)
- Enter data and save file again
- Email as attachment (or) print and then fax
Forms may be emailed as attachments to
or you may print them and then
fax to either 1-87-SELFDESIGN (toll-free in U.S. & Canada) or to (604) 224-3662.
As soon as we receive your registration form, we will email an invoice to you via PayPal. The registration will be complete when your payment is processed and confirmation is sent to you.
